Veteran Support Partnerships Across New England

Veteran Pathways of New England (formerly Clear Path for Veterans New England) operates within a highly coordinated regional network of federal, state, municipal, healthcare, and community-based partners that strengthen our ability to deliver rapid, Veteran-centered housing stabilization and wraparound support services across Massachusetts and New Hampshire.

These partnerships help ensure seamless referrals, coordinated case management, and integrated care for Veterans facing homelessness, behavioral health challenges, benefits access barriers, and long-term housing instability.

Federal & VA Healthcare System Partners

Veteran Pathways maintains strong collaboration with the U.S. Department of Veterans Affairs (VA) across the Central Western Massachusetts, Bedford, and Boston Healthcare Systems.

As a primary federal SSVF funding partner and referral source, the VA works closely with our team to coordinate:

Embedded VA social workers and clinicians from HUD-VASH, HCHV, and VA employment services collaborate directly with Veteran Pathways staff to support warm handoffs and rapid housing placements for Veterans in crisis.

We also partner closely with the Brockton VA REACH Program to provide transitional support and post-discharge housing stabilization services.

State Partnerships

The Massachusetts Executive Office of Veterans Services (EOVS) serves as a primary state funding partner for emergency housing and Veteran stabilization programs. Through this partnership, Veteran Pathways aligns with statewide Veteran service initiatives while providing rapid-response support to Veterans and families experiencing housing instability.

SSVF, GPD & Transitional Housing Networks

Veteran Pathways collaborates extensively with regional VA SSVF and GPD housing providers to coordinate referrals, co-case management, and transitions between emergency, transitional, and permanent housing programs.

Homeless Response Systems & Housing Authorities

Veteran Pathways is fully integrated into regional HUD Continuum of Care (CoC) coordinated entry systems across Massachusetts and New Hampshire, helping connect homeless Veterans with emergency and permanent housing resources quickly and efficiently.

Municipal Veterans’ Services Partnerships

Local Veterans Service Officers (VSOs) play an essential role in identifying Veterans in crisis, assisting with benefits navigation, and supporting stabilization efforts. Veteran Pathways works closely with municipal VSOs across Central and Northeastern Massachusetts, including Leominster, Gardner/Lancaster, Fitchburg, Lawrence, and Tewksbury.

Through partnerships with Fort Devens, Hanscom Air Force Base, and regional Transition Assistance Advisors, Veteran Pathways supports service members and military families with housing assistance, employment resources, and transition support services.

Family readiness teams and school liaison programs also help address the unique needs of military-connected families with school-aged children.

Recovery, Behavioral Health & Healthcare Partnerships

Through partnerships with Fitchburg State University and Middlesex Community College, Veteran Pathways supports student Veterans through outreach, peer connection opportunities, and resource navigation services.

Community & Civic Partnerships

Veteran Pathways also works closely with organizations such as Habitat for Humanity, municipal outreach teams, chambers of commerce, homeless service providers, and groups like 22 Mohawks to strengthen crisis response, suicide prevention efforts, and community-based Veteran support systems across New England.
